Varicella

A 7-year-old boy presented with generalized polymorphous rash involving the trunk, proximal parts pf extremities and the oral mucosa (soft palate) of three days duration. Usually after 3 days most lesions in chicken pox will be covered with hemorrhagic crustation as evident in this patient.

Strawberry hemangioma

A three-month old male infant presented with a less than one centimeter red vascular nodule on the cheek appeared shortly after birth. Strawberry hemangioma may involve any part of the body, however most commonly is seen on the face and of variable size.

Lichen planus

A 59-year-old man presented with 9-month-history of darkly pigmented rash on the side of the chin and cheek. It has been treated with topical and “systemic steroids” !!! for long time. At presentation linear hyperpigmented slightly atrophic patch with some still elevated papules at the periphery. LP commonly leaves post-inflammatory hyperpigmentation on spontaneous or treatment-induced resolution.

Strawberry hemangioma

A 7-month-old male infant presented with a large oval nodular vascular swelling on the abdomen since birth. It has characteristic purplish color and with fissured surface having somehow peau d’ orange appearance. It has been treated with Timolol (beta-blocker) eye drops 0.5% applied twice daily and waiting the result.

Tinea cruris

A 28-year-old male presented with a severely-pruritic,  minimally-scaly red plaques involved both crural areas symmetrically of 6 months duration. The patient gave history of applying many potent and super-potent topical corticosteroids with transient partial improvements soon followed by rash extension and increased itchiness and redness. He has been treated with oral griseofulvin with combined topical Clotrimazole and Hydrocortisone 2.5% in cream base for 4 weeks with excellent outcome.

Nummular eczema

An 8-year-old girl presented with a solitary, disc-like, red scaly patch on one leg of three weeks duration. The border seems to be more active than the center of the lesion, however KOH examination of the scales was negative and there was positive family history of atopic dermatitis. Excellent response to mometasone furoate 0.1%  ointment applied twice daily for two weeks. DSC05205

Papular urticaria

A 3-year-old girl presented with two pruritic urticated papules with central puncta strangely involved both shins in a mirror-image like pattern of few days duration. Note the symmetry of lesions and as I often state that insect’s bite doesn’t succumb to logic so may take any pattern !!!

Acral vitiligo

A 35-year-old male presented with depigmented patches confined to the dorsal surface of both hands of more than one year duration. No other body site was involved and family history of vitiligo or other autoimmune disease was negative. Acral vitiligo is one of the most difficult-to-treat type of vitiligo.
