A 12-year-old male presented with two non-cicatricial patches of hair loss of 3 months duration. Confinement of hair loss to marginal scalp areas (near hair line) is termed Ophiasis which usually considered as bad prognostic sign in alopecia areata (may resist treatment).

Isotretinoin-induced paronychia
A 20-year-old female presented with severe nodulocystic acne was put on Isotretinoin (Roaccutane) 40 mg capsule per day. Two months later with dramatic improvement in acne she presented with painful lateral nail folds paronychia confined to the middle fingers of both hands. Paronychia is an infection of one or more nail folds, usually with Staphylococcus aureus. It can be particularly stubborn and resistant to treatment but settles once the isotretinoin has been discontinued.
Cutaneous leishmaniasis
A 32-year-old woman presented with an asymptomatic, large, red, oval, indurated ulcerative plaque with punched-out border on the extensor surface of the forearm of 4 months duration. Intralesional injections of sodium stibogluconate given at 2 weeks intervals resulted in cure with some residual atrophic scar after 8 sessions.
Basal cell carcinoma
An 80-year-old man presented with a slowly growing oval ulcer covered with bloody-purulent crust and has rolled up border with easily bleeding tendency of three years duration. It has been excised and primarily sutured under LA with excellent results. Three years later on follow up there was no recurrence.
Squamous cell carcinoma complicating LSC
A 45-year-old man presented with long history of lichen simplex chronicus on the right lower leg of > 5 years duration. At last one and a half year an ulcer has developed and progressed slowly. On examination, an ulcer about 12 x 7 cm with an irregular outline, clean red base and punched out border surrounded by thickened dyspigmented skin was seen. Incisional biopsy revealed SCC.
Nodulocystic acne
A 20-year-old male suffered severe from of acne vulgaris for > 4 years. He presented with many inflammatory papules, nodules, cysts and scars involved the cheeks, chin and forehead. A 3-month-course of Isotretinoin (Retane) 40 mg capsule once daily resulted in fantastic improvement in spite of moderate-severe xerotic mucocutaneous side effects affected mainly the lips and to a lesser extent the forearms.
Steroid acne
A 21-year-old body-builder male presented with monomorphous papulopustular rash involved mainly the upper trunk and upper arms of three months duration. He gave history of anabolic steroids intake 1-2 months before the onset of the rash.
Tinea barbae
A slightly itchy red scaly rash involved the beard area with minimal hair loss of 6 months duration. The scales took a polycyclic configuration. KOH examination of the hair and scales were positive for fungi. Very good result on oral griseofulvin was obtained.
