Alopecia totalis

A teenager girl presented with alopecia totalis of one year duration with failure of many trials of topical therapeutic agents. She has been treated with monthly sessions of intralesional corticsteroids 5 mg/ ml. Hair regrowth started after first session and about 60-70 % of hair resumed after 3 sessions. However resumed hairs were of two colors black and gray.       

 

Ulcerative BCC

A 65-year-old man presented with an asymptomatic, rounded, 2 centimeters in diameter partially-crusted ulcer with a thread-like border involving the side of the face midway between the lateral canthus and the ear tragus of three years duration. Incisional biopsy confirmed ulcerative BCC. It has been excised with a suitable safe margin and primarily sutured.

Recurrent BCC

A 75-year-old woman had history of an ulcerative BCC which had been excised and primarily sutured three years ago. Now she presents with an asymptomatic, oval, superficial ulcer with somehow pigmented thread-like border at the same site of previously removed BCC. Again, it has been excised with 5 mm safe margin and primarily sutured.  

Steroid face

A 67-year-old woman presented with facial skin-epidermal atrophy (thin skin), persistent erythema, telengiectasis, purpuric spots and mutiple milia. Most of these manifestations were due to prolonged use of moderate-potent topical corticosteroids due to presumed chronic photosensitivity.

Von Recklinghausen’s disease

Asymptomatic, mutiple, skin-coloured soft tumors that can be pushed down into the panniculus by light pressure with the finger (“buttonholing”) and spring back when released. These nodules covered most of body areas and and their onset dated to early childhood. The father also has neurofibromatosis type I.

Black hairy tongue

A 28-year-old male presented with an asymptomatic blackish-brown pigmentation on the dorsal surface of the tongue of few months duration. The patient is smoker and has used to get large doses of oral antibiotics for dental problem.  The pigmentation can’t be rubbed off with tooth brush. The patient was instructed to avoid smoking , stop the current antibiotics, together with H2O2 mouth wash.

Onychomycosis

A 36-year-old house-wife woman presented with an asymptomatic, cosmetically-disfiguring nail discoloration involved all fingernails of many years duration. On examination: yellowish greenish nails’ discoloration particularely at the lateral borders and the base of the nail plates, paronychia and longtudinal split of the middle fingernail of the right hand. Nail clippings with KOH mounting showed fungal hyphae and spores and short budded hyphae and culture on Sabouraud’s agar yielded growth of both Candida albicans and Trichophyton rubrum. A prolonged course of  oral Terbinafine (Lamisil) 250 mg single oral daily dose for 4 months gave good result.

Cicatricial alopecia

A 24-year-old female presented with patchy alopecia of three years duration. On examination, a large oval area of cicatricial alopecia  with small red slightly scaly papules at the periphery of the bald patch were seen. No other body areas were involved. Skin biopsy taken from the periphery of the bald patch showed changes consistent with discoid lupus erythematosus.
