Obesity-associated acanthosis nigricans

An 18-year-old over-weight female presented with asymptomatic, thickened dark brown velvety textured patches of skin involved the sides and back of the neck of three years duration. No associated cutaneous, systemic or malignant disease.

Bullous impetigo

A 3-year-old female toddler presented with 5 days history of multiple small bullae superimposed on erythematous bases on the dorsal surface of the hand and nearby forearm. Some lesions have been ruptured leaving erosions; however, trails of blisters at the periphery of the lesions were seen on close examination. The child was a febrile and had no systemic symptoms. All lesions were cleared on one week course of oral cloxacillin.

Epidermoid cyst

A dome-shaped, skin-colored, about 2 cm diameter, asymptomatic nodule with surface telangiectasia in the center of the right cheek of more than one year duration. On clinical background, the main two differential diagnoses were epidermoid cyst and giant mulloscum contagiosum.  The lesion has been removed in toto under local anesthesia and found to be an epidermoid cyst.

Porokeratosis of Mibelli

A 21-year-old male presented with multiple asymptomatic circinate plaques on both sides of the nose of 4 months duration. The patient is bothered by the cosmetic appearance of the lesions. On examination, the lesions were well demarcated, circular plaques surrounded by slightly elevated borders (keratotic collars) and atrophic centers i.e. they have festooned appearance.

Basal cell carcinoma

A 40-year-old dark-complexioned man suffered a slowly developed ulcer on one side of the scalp. The ulcer has rolled up border and granular base. The patient complained that his lesion bleeds easily even on minor touch. Within 4 years the ulcer has reached about 7 cm diameter and resulted in localized hair fall.

Fordyce angiokeratomas of the scrotum

Multiple, small-sized, asymptomatic, purplish papules with a slightly scaly surface studded the scrotum of a 35-year-old man for more than three years. The patient has been reassured regarding the benign nature of these lesions. 

Melanocytic nevus

A 26-year-old male presented with sudden appearance of an asymptomatic, worrisome, blackish papule on the forearm of one month duration. O/E: an irregualr-outlined dark-colored papule about 6 x 4 millimeters in greatest diameters. It has been removed in toto and sent for HP examination. Biopsy showed benign looking melanocytes at the dermoepidermal junction.

Epidermoid cyst

Epidermoid cysts often incorrectly called ‘sebaceous cysts’ are common and can occur on the scalp, face, behind the ears and on the trunk. They often have a central punctum; when they rupture or are squeezed, foul smelling cheesy material comes out. The present case is a an 85-year-0ld manpresented with a dome-shaped epidermoid cyst in the epigastric region of more than 10 years duration. It seems that the skin overlying the cyst has been atrophied with some dilated vessels due to the pressure effect produced by the cyst. The punctum in the central part of the cyst is so evident.
